FIB-4 calculator

Estimate the likelihood of advanced liver fibrosis from age, platelets, AST and ALT, without a biopsy.

Your result appears here as you type.

The formula

FIB-4 = (age × AST) ÷ (platelets × √ALT). AST and ALT are in U/L, and platelets in 10⁹/L.

Reading the score

Cut-offs vary by liver disease and guideline. In people aged 65 and over, a lower cut-off of 2.0 is often used. Hepatitis flares and other causes of raised AST can distort the score.

FIB-4Meaning
Below 1.3Advanced fibrosis unlikely
1.3 to 2.67Indeterminate, consider further tests
Above 2.67Advanced fibrosis more likely

Common questions

Still unsure? Send us the question and a person — not a bot — will answer it.

What is a normal FIB-4 score?

There is no single normal value. Below 1.3 suggests advanced fibrosis is unlikely.

Can FIB-4 diagnose cirrhosis?

No. It is a screening score. Follow up a raised result with elastography or specialist review.

Is FIB-4 useful for hepatitis C?

Yes. It was first developed in people with HIV and hepatitis C coinfection, and it is widely used in viral hepatitis and fatty liver disease.
