What a good referral letter says

  • Who you are, and how the receiving doctor can reach you.
  • The question you want answered, in the first line.
  • The relevant history and examination findings.
  • Tests already done, so they are not repeated.
  • Current medicines and allergies.
  • How urgent the referral is.

Getting it right

  • Keep it to one page where you can. Busy consultants read the first lines.
  • Send copies of important reports with the patient.
  • Tell the patient why you are referring them, and what to bring.
  • Keep a copy in the patient’s record.
