Patient consent form maker

Create a clean procedure consent form with your clinic’s details and download it as a PDF. Leave fields empty to print blank forms. The signature records the conversation with the patient; it does not replace it.

Leave any field empty to print a line and fill it by hand. Nothing you type leaves your browser, and only your clinic’s details are saved on this device.

Spoken consent covers a routine examination. Get it in writing for surgery, anaesthesia, invasive or risky procedures, and whenever you plan to use a patient’s photographs or records for teaching or publicity.

What to explain before the patient signs

Explain it in a language the patient understands, in Urdu or a regional language if needed, and note that on the form.

  • What the procedure is and why you recommend it.
  • The expected benefits and the risks that matter.
  • The alternatives, including doing nothing.
  • Who will do the procedure.
  • That the patient can ask questions, and can refuse or change their mind.

For patients: before you sign

You have the right to understand what you are agreeing to. Before you sign a consent form at a hospital or clinic, ask:

  • What exactly will be done, and by whom?
  • What are the main risks, and how common are they?
  • What happens if I do not have it? Are there other options?
  • Can someone explain it to me in Urdu or my own language?

Minors, and patients who cannot sign

  • For a child, a parent or legal guardian signs.
  • For an adult who cannot decide, a close relative usually signs. Write down who they are and why the patient cannot.
  • If a patient cannot write, take a thumb impression with a witness.
  • In a life-threatening emergency, record why treatment could not wait.

Last checked 31 October 2026. This is a general sample, not legal advice. Adapt it to your practice, and ask your adviser or provincial healthcare commission when a case is unusual.

Common questions

Still unsure? Send us the question and a person — not a bot — will answer it.

Can I print blank consent forms?

Yes. Enter your clinic name and leave the rest empty. Every empty field prints as a line to fill by hand.

Is this form legally approved?

No. It is a general sample. Check it with your adviser or provincial healthcare commission, and add anything your procedures require.

Do I have to sign a consent form as a patient?

No one can force you. You can ask questions first, ask for more time, or refuse, though the doctor may then be unable to do the procedure.

What is informed consent?

It means the patient agrees after being told, in words they understand, what will be done, why, what the risks are and what the alternatives are.

Is a signed form enough on its own?

No. The form is evidence of the conversation. If the risks were never explained, a signature does little to protect you or the patient.

Does a consent form need a witness?

A witness is good practice, and especially important for a thumb impression. Check what your hospital or provincial healthcare commission asks for.

Where should the signed form be kept?

In the patient’s file. In SehatDoc you can upload a scan to the patient’s documents, so it stays with their record.
