What every medical certificate should include

For patients: how to get a medical certificate

Only a registered doctor can give you a medical certificate, after examining you. Visit a doctor while you are unwell, and ask for one for your office, school or college. Check whether your employer needs its own form or a certificate from a particular hospital. Never buy or make a certificate yourself: a false certificate can cost you your job or admission, and it can lead to legal trouble.

Good practice

• Issue a certificate only after you have examined the patient yourself.
• Certify only what you found. Never backdate a certificate or write dates you cannot support.
• Leave the diagnosis out unless the patient agrees, because the employer or school will read it.
• Keep a copy in the patient’s record.
